I'm 1,85 m / 86kg with relatively wide shoulders. My jacket size is something between 42 and 46. I depends on the maker and the jacket type. For example: my eastman A2's are usually 42 but my navy cloth jackets are more 44, and in the case of AVI's M422a I have a 44 and it fits more like a A2 fit, if I would more go after the typical ww2 navy look I would have been better with a 46.
If you order from Morten make sure that you custom size the sleeves.

Does AVI also produce the 421 jacket?
I agree with B-Man2. WPG offered their version some years back (15-20 years ago?). It was labeled as "M-421B". I have 3 originals. 1 from H.L.B. Corp and 2 Blocks. The WPG comes close to the feel of the originals and WPG offered them for under $100. I bought one and liked it so much I bought 2 more. There are a couple on ebay from WPG now. While they no longer offer them, if you can find one from “What Price Glory” they had decent looking repros . You can sometimes find them on EBay .
